BOYNTON BEACH, Fla. -- A Boynton Beach police officer who was injured when the police cruiser she was driving crashed into a concrete pole had been disciplined before for other crashes while on the job, police documents show.Officer Rachel Loy was one of three officers injured Tuesday morning when the police cruiser they were traveling in crashed into a concrete pole during a chase. The pole snapped and landed on top of the vehicle, police said. According to Boynton Beach Police Department internal affairs documents released Wednesday, Loy was disciplined twice in 2007 for getting into two crashes with department-issued vehicles within a six-month period.Loy's first crash occurred in June 2007 when her cruiser was struck by another car at an intersection while responding to a call. According to the documents, Loy's supervisor said it was "clearly evident" from watching the cruiser's dashboard camera video that she "attempted to proceed through the intersection without first ensuring that the intersection was clear." She was given a written reprimand.

Then in October 2007, Loy was behind the wheel of her cruiser when she struck a garbage can, shattering the passenger's side rear-view mirror and causing $75 in damage. Both crashes were deemed preventable, and Loy was suspended for one day after the second crash.Loy, Officer George Baldino and Officer Clayton Harris were providing backup to a burglary call at the time of the crash Tuesday.Police said neighbors noticed three men going inside a house a few blocks away and called 911. When police arrived, the men sped off in a truck and led officers on a brief chase along Seacrest Boulevard before crashing into another house. All three men fled after crashing. Police caught two of the suspects -- Michael Hollis and Chedo Ceance -- almost immediately, but the third suspect got away. Police said Markell Hollis is considered armed and dangerous. All three officers were taken to Delray Medical Center. Harris, who also has a disciplinary record, was released from the hospital late Tuesday.
